Note: Printing reimbursements must be done at the DM Office by a full time staff member. Please do not fill out a Problem Report Form in the case that you need a reimbursement.

Please refer to the Sticker Tag located on the top of the CPU
Please indicate the printing or Plotting device that is having problems.

Click on the Submit Button and this form will be E-mailed directly to the DM Staff. Please allow at least 24 hours for the problem to be addressed. Requests will be handled on a priority bases. If you have any questions, please contact the DM Staff.
